- Bay Gardens Funeral Home will notify Incoming Securities on behalf of any deceased person who is receiving Canada Pension Plan (CPP) or Old Age Security (OAS).
- Application for CPP Death Benefits and CPP Survivor Benefits
- Information on Old Age Security Pension Allowance: A monthly benefit is available for low-income widowed spouses or surviving common-law partners who are between 60 and 64 and meet the residence requirements.
- Information on Guaranteed Income Supplement: The Guaranteed Income Supplement provides extra money for Old Age Security Pensioners with a low income.

How to Cancel OAS/CPP Benefits: When an Old Age Security (OAS) and/or Canada Pension Plan (CPP) pensioner/beneficiary dies, their benefits must be cancelled. Benefits are payable for the month in which the death occurs; benefits paid after that will have to be repaid.
This includes the following benefits:
- Old Age Security Pension, including Allowance, and Allowance for the survivor
- CPP retirement pension
- CPP disability benefits

- Veteran Allowance: The Last Post Fund may provide assistance to Canadian Armed Forces veterans without sufficient funds for funeral expenses. Benefits conditional based on certain requirements.
- GST/PST Credit: Any GST & PST credits received by the deceased in the month of the death may be kept, however, benefits received in the month(s) following the death must be returned.
- Child Allowance & Child Tax Benefits: If child allowance or tax benefits were received by the deceased, advise CCRA Tax Centre of the death and inquire if the deceased’s spouse or the guardian of the children are eligible for the benefit.
- Final Income Tax Return: Complete a Terminal tax return. Once the Notice of Assessment is received, apply for a clearance certificate.
- Ontario Ministry of Health (Termination of Health Coverage): To cancel a health card you need to mail a photocopy of the death certificate along with the health card to the address below. If you do not have the health card, but you do have the health card number, you may write the number on the death certificate before mailing it in.
